Greg Garra's 5x7 inch booklets that the residents carry.  Each booklet contains 18 pages with forms 1-8.  At the end of a shift the residents will ask attendings to complete the form which will be handed in to the program coordinator at the end of the shift.  The program coordinator will enter them into cordtests manually or one can use an optic scanner to place the information into an excel file.
